What is a form f?

Once you have completed your application form, the next step will be the Form F assessment. The Form F is a way to document your assessment. It will be used when you reach the final stage of going to panel.

The Form F assessment is a lengthy process but you will be assigned a social worker, who’s job will be to support you every step of the way.

It might take a few months to complete the Form F assessment, but once you have, it will be time for the final stage – going to panel. It is when you attend panel that it is decided whether you should be approved to be a foster carer.
